Referring doctors may refer patients through our website by logging in.  Just follow the link to the left for “Referring Doctor Login”, but if you prefer, you may download and print out our “ZVE REFERRAL SLIP” for your office.  Please indicate if you feel this case is appropriate for consultation only (such as difficult diagnosis, questionable prognosis, cracks, resorption, etc.), as opposed to same day consult and treat.  If provided information on the tooth’s restorative plan we can better prepare cases for ongoing treatment.
